Prepare for your first appointment with Dr. Basques. A well-prepared visit ensures you get the most out of your consultation and helps us provide the best possible care.

How to Prepare
Before Your Appointment
- Complete any intake or health history forms sent to you ahead of time — this saves time at check-in.
- Write down all your symptoms: when they started, what makes them better or worse, and how they affect your daily life.
- Gather all prior imaging (MRI, CT, X-rays) on disc — we need the actual images, not just the written reports.
- Make a list of all treatments you have tried: physical therapy, injections, chiropractic care, medications, and whether they helped.
- Check with your insurance company about coverage, referral requirements, and any out-of-pocket costs.
Travel & Accessibility
- If you are traveling from out of state, plan to arrive the day before your appointment.
- Our offices are wheelchair accessible with ground-level entrances and accessible parking.
- If you need assistance getting from your car to the office, our staff is happy to help — just call ahead.
- For patients who have difficulty sitting for long periods, please let us know and we will accommodate you promptly.
What to Expect
Check-In
You will check in at the front desk with your ID and insurance card. New patient paperwork will be completed if not done in advance. Please arrive 15 minutes before your scheduled appointment time.
Medical History & Intake
A member of Dr. Basques' clinical team will review your medical history, current symptoms, prior treatments, and medications. Your imaging studies will be loaded into our system for Dr. Basques to review.
Physical Examination
Dr. Basques will perform a focused neurological and musculoskeletal examination, including strength testing, reflex assessment, sensation testing, and gait evaluation to correlate with your imaging findings.
Discussion & Treatment Plan
Dr. Basques will review your imaging with you in detail, explain your diagnosis in clear terms, and discuss all treatment options — both non-surgical and surgical. You will leave with a clear plan and answers to your questions.
Important Note
If you are scheduled for a procedure and have not yet received pre-operative instructions, please call our office at (401) 443-4210. Do not stop any prescribed medications without first consulting the prescribing physician.
